During my professional career as a clinician for nearly five decades, I have come across several instances where felt that role has been failure. consider these critical incidents molded clinical practice and hopefully made me better clinician. In this paper, will discuss about few of those situations, narrating the experience lived through, highlighting think went wrong how best we clinicians should perform under such circumstances. divided paper into lapses in history taking, lack completeness physical examination, premature closure making diagnosis inadequacies treatment, each be illustrated using real-life scenario through. As clinicians, strive zero error, but human beings are imperfect often room improvement ways act react. your practice, irrespective whether you or not, it is always to reflect could treated patient seen, make more reflective rather than thinking stereotyped manner.
